On 11/20/12, at 1:46 pm, a self reported incident was called in by Charles Curet with Phillips 66 about a sheen discovered at the facility's dock located in Clooney Loop of the Calcasieu Rover. Work was being done at the doc and creosote pilings were being removed. The facility anticipated sheen being emitted from the pilings upon removal and had contacted the USCG prior to the start-up of this project. USCG MST 2 Meritt had instructed the facility to make an initial report the first time sheen was produced, but not to worry about reporting any additional sheening created by this project as long as it was contained. On 11/26/12, they confirmed the reporting strategy given to the facility and stated that the area is and will remain contained with boom, and ES&H is and will remain at the scene until the project is completed (approximately 3 weeks) ES&H used absorbent pads to remove sheen from the water. Material went offsite and was released to surface water. No off-site protectice action was taken.

Cause: TDC was preparing a line for maintenance by isolating and purging it, and during this maintenance activity, TDC sent a high concentration of hydrogen sulfide containing gas to the South Flare System. Sudden spikes in flow from the third party NASH plant of a stream with high hydrogen sulfide concentration and sufficient flow overwhelmed the capacity of the South Flare Caustic Scrubber to adequately treat the material.
