Cause of Problem: Equipment Design

Fault in tank floor caused a leak in the bottom of the tank.
Water was pumped into the tank in an effort to keep the naphtha level in the tank above the leak. A vacuum truck and pumps were deloyed to recover spilled naphtha on the gound. Contaminated soil was excavated and disposed of off site. No samples taken by LDEQ investigator.

O-ring in a soleniod valve failed. Red kerosene based dye sprayed onto dock and into water.
Red dye cleaned from dock and attempt to recover the dye spilled to the water with sorbant pads was made.
