Cause: While working on the Alky splitter tower reflux pump, the technician loosened screws from a mounting plate connected to a pressure gauge. The tubing connection located upstream of the isolation valve failed and released hydrocarbon. The initial report from the facility stated that 178 pounds of propylene were released when a 1/2 inch tubing broke a reflux pump, but the release was later determined to be below reportable quantity.
