|#4 Vac Unit - Distillation Unit 300-350||Cause: Pump seal failure on a viscosity distillate pump for the vacuum distillation unit 300-350. Hot oil sprayed out and flashed (smoke burn). #4 vacuum unit.|
Notes: No Information Given
|SRU #1||Cause: Air demand lines for the SRU were plugged and caused the unit to shut down. SRU tripped, incinerator smoking for 20 minutes; permit exceeded (permit allows for maximum of six minutes)|
Notes: Workers have cleared the line and the unit is back on line; there is no reportable level of sulfur dioxide released
|#4 Vac Unit||Cause: Flange failure on 700 pumparound in the #4 vac|
Notes: Extinguished fire, shout down the unit. The flange was replaced and a root cause analysis will be performe
|#4 Vac Unit||Cause: A six inch pipe on the unit failed and spilled oil on the unit, resulting in fir|
Notes: The plant shut down with material going to the flare. The report notes a lot of smoke but says airborn pollutants were below reportable quanitities.
|Tank Farm||Cause: Tank 92 fire inside the tank. Black smoke was visible. 5 to 8 feet of hard pin asphalt was being heated in order to transfer from damaged tank. Fire occurred because heater tubes were exposed from asphalt due to low product level in tank.|
Notes: Extinguished fire.
|FLARE||Cause: LDEQ states that the plant lost power, causing a loss of steam to the flare. This caused excess smoke to be released from the flare.|
Notes: Issue was resolved when power came back on. LDEQ states that no offsite impact was noted. LDEQ report only. No refinery letter.
|No LDEQ Reported|
|Cause: Over pressurization of #5 Boiler.
High BTU liquid hydrocarbon present at boiler #5 lead to an explosive mixture in the boiler. Fire and smoke were witnessed during boiler start-up.|
Notes: Systems stopped and re-started several times. Instrument technician was called in to perform repairs.
|Sour crude unit #1 Atmospheric Tower Bottoms pump||Cause: Fire in the No. 1 Sour Crude unit due to failure of Atmospheric Tower Bottoms pump.|
Notes: Isolate unit and remove feed to stop fire. Use fixed monitors and deluge system to contain. Repair pump. Field interview was conducted by LDEQ on 5/27.
|Platform Unit Charge Heater #1 (EQT 250)||Cause: Rupture of tube in heater fire box. No 1 Platformer charge heater (EQT 0250) exceeded opacity >20% for approximately 20 minutes. The unit was turned off and cooled.|
Notes: Feed and fuel into heater were removed, steamed out and water was sprayed to cool. The report states that the heater tubes will be repaired to proven recurrence. A field interview was conducted on 5/23 and photos were taken which are included in the pdf file.
