see notes. A loss of electrical power occurred caused by failure of an Entergy own electrical transformer. The electrical power loss resulted in emergency shutdown of the majority of process units. Emergency shutdown instrumentation directed hydrocarbon gas streams to the refinery complex flares resulting in excess emissions of SO2, H2S, and NOx from the complex's North and South Flares. Additional flaring of gas streams resulted from the staggered process unit and various start up once electrical power was restore
