LDEQ Notes: According to Calumet, they were unaware of any malfunctions or incidents. The facility has a new hydrotreater that may have caused the flare to "burp", using hydrogen and propane.

Description: Ms. Young called at 2:24pm to report smoke and flaring.

LDEQ Notes: Mr. Williams had called at 2:15 that day to inform LDEQ that the #5 boiler was shutting down due to lack of steam. This caused a large amount of black smoke and flaring. Mr Williams called at 2:30 to report that #5 boiler was back in service.
