4 inch gate valve packing blew out releasing hydrogen into the atmosphere. Incident cause: the packing blew out the 4 inch valve
Roads east of the refinery were temporarily blocked as a precautionary measure. Also LOHT was shutdown and depressured to the flare.

Pump seal failed on tank 164 and leaked onto electrical heat trace. Electrical ignition of heated oil started a fire and plant personnel extinguished the flame.
