Accident Classified As: Reportable Quantity
On Friday, March 14, 2014, at approximately 1707 hours, Chalmette Refinery personnel observed a small visible sheen at the crude terminal dock from utility pump G-2907. A small leak developed on G-2907 and was leaking into the containment area beneath the pump. A small hole had developed in the containment area allowing a small amount of diesel to leak into the river.
Inspection data less than adequate to provide early indication of the containment failure.
After isolating the leak operators used absorbent pads to remove hydrocarbons from the pan and deployed initial containment boom into the river. USES were utilized to clean the affected decking and pilings, collected the sheen, and removed the boom. The catch pan below G-2907 will be repaired or replaced.
