A company-owned tug boat, the M/V Kentucky, was moored onsite, shifted and listed during extreme low tide, and thus flooded the engine room causing the spill.

LDEQ-SPOC was notified of oil sheen on the Calcasieu River near Phillips 66 Dock 3 at 7:57am on 11/25/12. They initiated investigation with immediate reponse by site visit and determined that the source of the sheen was not Phillips 66, but rather was from nearby Mike Hooks, Inc. (dredging company) diesel spill that had earlier been reported to LDEQ to have occurred at 6:13 pm on 11/23/12 (incident # 144913). Cleanup and recovery of the diesel was initiated shortly after the spill occurred and was near completion at the time of the site visit on 11/25/12. Containment booms and skimmers were deployed. The diesel cleanup/recovery was performed by a contracted third party, ES&H. The USCG was onsite and was supervising cleanup and recovery. The company initially reported approximately 15 gallons diesel spilled. During the site visit and from the follow-up notifications the final quantity spilled was determined to be approx. 768 gallons diesel fuel.
