The packing on the hydrogen valve blew out. According to the refinery letter, "the valve had been closed... and was opened for startup of the unit. The valve packing was not checked or replaced during turnaround activities."

BRQ. Princeton personnel dumped unit pressure to the unit flare and nitrogen was introduced into the system. The Princeton fire brigade "responded and directed fixed monitor streams and advanced hose teams to protect the surrounding equipment." It was recommended to replace valve packing for every future planned shutdown.
