Refinery cooling water tower CT-405 experienced a valve malfunction which resulted in a loss of cooling water to our Naphtha Hydrotreating unit (NHT). Due to the lack of cooling water, our NHT stripper overhead pressure increased and caused PSV-39-235 to relieve to the flare. Simultaneously when the CT-405 valve malfunction, the recirculating cooling water was released from the basin to the ground and flowed offsite.
