Cause of Problem: Power Failure
Lost primary and alternate power to instrumentation control processers for East Plant. Control valves and other I/O failed to their last recorded position during the power outage. When power was restored, control valves and other I/O returned to their pre-programmed settings causing some process upsets and equipment shutdowns.
