an unexpected heater tube rupture resulted in a fire at HTU-1
As described above, the release resulted from an unanticipated tube rupture in the convection section of the heater. While the investigation is ongoing, Motiva currently believes that this rupture could not have been reasonable anticipated; Motiva was following normal inspection and operating procedures prior to the incident.
Motiva is thoroughly inspecting the heater and other affected equipment and will make necessary repairs prior to restarting the heater and process unit. Will re-evaluate inspection frequencies, preventive maintenance frequencies, operating limits and associated procedures, and make necessary changes to help prevent recurrenc
