155952 2014-05-12	LUI West Tank Farm, Tank 20D-2	Cause: Waste water containing ammonia salts and phosphoric acid leaked through a roll-off box and a secondary-containment unit that had a small hole. The box and the collapsible secondary-containment unit were themselves inside an earthen containment. Both the primary and secondary containment systems failed in this case. The incident occurred at the LUI West Tank Farm, Tank 20D-2.
