Transformer had been taken out of service to repair a secondary busing oil leak, and during re-energizing of the transformer the malfunction caused a power outage, causing shutdown on several critical pumparound pumps; the resulting loss of heat removal caused the overheated drum F-210 to fill with liquid.

Cause: On Feburary 12, CITGO Lake Charles Manufacturing Complex notified LDEQ's Single Point of Contact of a sulfur dioxide release that occurred on February 11, 2014 at 20:18 hrs. According to Phyllis Holifield, Manager of Environmental Protection Department, stated that an amine pump tripped out in the Fuel Gas Processing Unit stopping the amine flow for 4 minutes which lead to the release of sulfur dioxide from various furnaces in the unit.
