Notes: Leak discovered @ Reformate Tank 85 coming from 2 small holes immediately above the wind stiffener ring at the 36.5 foot level; Verbal report indicates failure of gauging system contributed to late detection of leak. REMEDIAL ACTION: A new safe operating fill height has been established below prior level. Tank 85 will be replaced. High level alarm has been lowered to prevent recurrence.

Cause: A release of reformate and benzene to the soil due to a leak on a blend line. The line was 14 diameter and was being excavated to locate the leak location.

Transfer Line
Cause: During maintenance activities of a transfer line, Reformate was spilled to the ground outside of containment. The transfer line was being drained from a line break at a flange. When the line break at the flange was made, a rapid release of Reformate overcame the secondary containment. No off site impacts due to this spill. Reformate is a mixture >/= 80% Naphtha, 10%-<35% Toluene, <1% Benzene

Followup: Yes

Notes: The transfer line flange was re-bolted to stop the release. Pressure gauges were installed on the transfer line to prevent a rapid release of Reformate while daining the line during the maintenance activity.
