Cause: The crude butadiene import drum PV-2170 overfilled while crude butadiene was being imported at the facility's wharf. The flare knock out drum PV-191 filled with crude butadiene resulting in flaring from Shell Chemical West Operations Elevated Flare. FLARE.

Notes: Operations immediately stopped the crude butadiene import in order to stop the overfilling of PV-2170, under investigation. Refinery letter dated 10/14/10 states that follow-up letter is forthcoming, but no follow-up is included in file. No LDEQ report. File includes letters from Shell Chemical and Motiva Refinery.
