Cause: The SRU was regenerating catalyst and during the recovery process, the material was released. Suspected that the stack flew meter is measuring high but until confirmed there may be a catalyst regeneration issue in the SRU.

Cause: EXPLOSION of Belco tail gas SRU (sulfur recovery unit) "resulting in a long term loss of the scrubber" Report lists no releases/emissions, states that there were no injuries. FIRE.

Notes: BRQ. "Belco unit isolated. area barricaded. all sulfur producing units were idled in a controlled manner" Off site impact: "Some residential damage from explosion shockwave"; no environmental impact noted.

Cause: Light lube oil Calpar 80 release of 18 barrels (from storage tank 10) was reported on 9/25/2012.

Notes: Contractors removed impacted materials outside of secondary containment and vacuumed fluids inside of secondary containment. The facility has been doing weekly inspections of the secondary containment structure and has noted planned discharges of stormwater.		Lube Oil: 756.0 gallons

Cause: A fuel gas explosion destroyed boiler #5 and caused an evacuation. The cause of the explosion is unknown and under investigation.

Notes: Air monitoring was conducted and no harmful emissions were detected. A root cause analysis will take place and findings will be sent to NWRO. There is no incident report and no SPOC report attached to the file. The monitoring report form is attached to this file.
