82252 2005-09-15 | Platformer | Cause: Exchanger head gasket failed. Platformer feed effluent exchanger was the unit the leak developed on. Burned hydrogen and flammable hydrocarbons as they were released to the atmosphere. Followup: No Notes: Personnel extinguished fire with water, dry chemical and depressured the operating un | Hydrogen: 10.0 pounds Fuel: 246.0 pounds |
80881 2005-07-29 | #4 Vac Unit - Distillation Unit 300-350 | Cause: Pump seal failure on a viscosity distillate pump for the vacuum distillation unit 300-350. Hot oil sprayed out and flashed (smoke burn). #4 vacuum unit. Followup: No Notes: No Information Given
76443 2005-01-25 | Tank Farm | Cause: Flange failure. A flange in the pipe rack located northwest of T-98 failed, releasing about 5 barrels to the surrounding ground. It was contained in the tank ber Followup: No Notes: Directed vac truck to vac the spill up and recycle the product back into the refinery. No specific remedial action planned to prevent recurrence. Flange failures not common cause of spills. | Atmospheric Tower Bottoms (Crude Oil): 210.0 gallons |
87642 2006-05-04 | LOHT | Cause: 4 inch gate valve packing blew out releasing hydrogen into the atmosphere. Incident cause: the packing blew out the 4 inch valve Followup: No Notes: Roads east of the refinery were temporarily blocked as a precautionary measure. Also LOHT was shutdown and depressured to the flare.
No LDEQ Reported 2006-03-18 | Tank 164 | Cause: Pump seal failed on tank 164 and leaked onto electrical heat trace. Electrical ignition of heated oil started a fire and plant personnel extinguished the flame. Followup: No Notes: Fire extinguished by plant personnel. No injuries or offsite impacts. No specific remedial action taken. | Paraffinic Lube: 2.0 gallons |
86121 2006-02-10 | Flare Gas Compressor - FL 2001 Same as above | Cause: Broken valves in FGC were repaired during and emergency shutdown. Followup: No Notes: Valves were repaired upon shutdown. | Sulfur Dioxide: 11,880.0 pounds Hydrogen Sulfide: 126.0 pounds |
94730 2007-03-20 | #4 Vac Unit | Cause: Flange failure on 700 pumparound in the #4 vac Followup: No Notes: Extinguished fire, shout down the unit. The flange was replaced and a root cause analysis will be performe

No LDEQ Reported 2008-01-05 | Tank Farm | Cause: Pump seal failure on isomerate tank Followup: No Notes: Release was below reportable quantities. Used water and an ABC fire extinguisher to put out fire. Isolated pump from product. | Isomerate: 30.0 gallons |
112725 2009-02-13 | Diesel Hydrotreater | Cause: Flange seal leak caused a fire in the diesel hydrotreater unit on the reactor feed effluent exchanger flange on the second deck. Followup: No Notes: no information given
148795 2013-05-20 | crude distillation unit | Cause: On May 20th, Calumet Lubricants Shreveport Refinery self reported a seal fire from a crude oil leak. At approximately 0830 hours a charge pump seal was to the crude distillation unit reported to have started leaking and caught on fire.
On May 29th, LDEQ spoke with Calumet's environmental engineer - he advised that the charge pump to the crude distillation unit developed a small leak, which caught on fire. The material was contained on the containment pad around the pump and was determined to be Below Reportable Quantity. Followup: No Notes: Material was contained on the containment pad around the pump. The seal was replaced and the unit was returned to service. Less than 1 gallon of oil discharged to containment pad around pump.
