Exchanger e-108: Fire
Cause: A leak at the gasket between the shell and the tube face plate on the exchanger caused super heated hydrogen to be released. The leak was due to loose bolts on the face plate. This caused a fire in the hydrogen ignited in the atmosphere and started a fire. E-108 on fire

Notes: Princeton personnel responded by dumping unit pressure to the unit flare and unloading the compressors. Nitrogen was introduced into the system. Princeton fire brigade responded and directed fixed monitor streams to protect surround equipment. Bossier Parish Fire District No. 1 responded to the incident to assist with hose operations. Fire lasted 57 minutes. Bolts were torqued to 80% bolt strength. It is recommended to modify the maintenance procedure to prevent future incidents. Material Safety Data Sheet for hydrogen included within the report. Atmospheric Venting Caused a fire.
