Cause of Problem: Equipment Failure

Loss of containment occurred due to the structural failure of check valve on the discharge of Heater H-11001, which allowed a hydrocarbon release to the atmosphere. The hydrocarbons ignited causing significant damage to the process piping and insturmentation associated with the heater
