Beginning at 1030 on 5/8/14, operations noticed the North Flare Stack SO2 analyzer was reading 20-30 lbs/hr. The source of emission was unknown at the time and the environmental department was notified. A courtesy notification was made at approx. 1930. The cause of the emission has since been determined to be a leaking relief valve (RV). This was an allowable emission under facility permit 2580-00013-V12. There were no known offsite impacts.
The relief valve malfunction was not an incident that could have been prevented. However, LABB researcher asserts that operations personnel should have had access to some form of alarm or alert aside from simply "noticing" the elevated SO2 levels at the stack.
Operations took multiple samples of the flare system to determine source of the material.
