Cause of Problem: Equipment Failure
Shell Chemicals OL5 Unit released low pressure stripper bottoms material (benzene) to the concrete slab from the bellows on a relief valve (RV) in OL5. Operations immediately blocked in the RV and began to clean up the spill.
