Cause of Problem: Equipment Failure

Someone noticed a temperature change in one of the processes in the plant, which led this person to check a relief valve. The relief valve, which is on top of the #1 Chloroform Storage Condenser, was leaking. Both of the possible chemicals released would have evaporated before they hit the ground. None of the air monitors 15 ft below the condenser went off.
