Accident Classified As: Below Reportable Quantity (BRQ)
First report letter states cause as routine shutdown/startup of the Sulfuric Acid Plant, and that no RQs were exceeded. Second letter for the same incident date claims the release was the result of a small hole found in the shell of the B-502 Reaction Furnace at the D-Sulfur Recovery Unit, and still stating that no RQs were exceeded. Third letter reiterates that it was a routine shutdown, but that RQs may have been exceeded.
Claims emission was below RQ in first two letters. All different dates for times of phone call notification (September 12, 13, and 14) and all assigned different case numbers.
