Cause of Problem: Equipment Failure
Safety valve on WCT 01 failed and released to flare. Root cause for flaring was positional failure of the control valve resulting in a pressure surge to the tower WCT01. The safety valve did not properly seal after the lift and VOCs leaked to the flare.
