The root cause was found to be operator error. The operator failed to secure the line during a unit shutdown to clean equipment. Thus, some temporary piping that had been put in and left open-ended upon disconnection.
"Conoco failed to diligently maintain control equipment in proper working order whenever emissions were being made which is an area of concern"
Corrective action was taken to isolate the open-ended pipe identified during the incident investigation.
