Cause of Problem: Equipment Failure

While neutralizing a tank located in the acid plant, residual nitrosylfuric acid reacted with water and caustic, releasing nitrogen oxides into the ambient air. Nitrosylfuric acid is formed as a result of combustion in the sulfuric acid furnaces and accumulates inside equipment and lines.
