Cause of Problem: Equipment Failure
The West Coker T-101 safety valve lifted when the gas balance line valves went into the fail close position due to the failure of the Programmable Logic Controller (PLC). A fault signal was received by the PLC card causing it to fail.

An unexpected fault signal was received by the PLC. This resulted in the failure of the PLC card.

The PLC card was reset.
