Normal operating overhead pressure was exceeded on the PHLA unit T-2 tower due to a pressure controller valve malfunction.
Not preventable due to unanticipated overpressure of the T-2 tower.
The bypass valve was opened to reduce tower pressure, the safety valve was reset, and the unit was returned to normal operating conditions. The pressure controller instrument was replaced immediately following the event. Only the reportable quantity for flammable vapor was exceeded.
