Operator on rounds discovered stain on nozzle bottle of vessel of hydrocracking unit. Plug missing, visible vapors, and there is a quarter inch hole leaking.
"Unforeseen equipment failure."
BRQ. Refinery letter states that "final calculations confirm that no reportable quantities were exceeded." Steam lanced the leak initially, then got advice from piping inspectors for leak repair. Repaired with nipple and isolation valve with cap.
