Cause of Problem: Seal or Gasket
Hydrogen Fluoride mixed with iso-butane, butane, and a seal barrier fluid sprayed from a quick connect on the seal pot of the main fractionator reflux pump onto an operator's left arm and shoulder. The injured operator was kept in a hospital overnight.
