A valve misalignment on the iso-butane pump allowed iso-butane to be routed to the flare for approximately six hours before being discovered. While troubleshooting the flaring incident, operations found the relief valve on th DHT unit's recycle gas scrubber "chatter" to the flare. during the investigation it was discovered that the newly installed valve had malfunctioned
the iso-butane release was preventable. The failure of the release valve, however, was not preventable, as it was not foreseeable.
