Cause of Problem: Instrument Failure
there was an air trip transmitter failure that caused the Sulfur-3 unit to trip online unexpectedly. Due to this online trip, a release of sulfur dioxide (SO2) into the air occurred from both the S-3 and S-2 tail gas incinerators .
