Accident Classified As: Reportable Quantity
Cause of Problem: Equipment Failure
Make-up hydrogen compressor (C-102B) malfunctioned which caused a higher rate of hydrogen into C-102 compressor. This increased pressure which resulted in a safety valve release and caused flaring.
Discharge Preventable - No
Incident occurred as a result of compressor malfunction.
Notes/Remedial Actions
Release exceeded SO2 RQ. The safety valve was removed from service. Pressure maintained by companion safety valve. Compressor will be repaired. Company report and State Police HAZMAT report differs on pollutants released.
