Cause of Problem: Equipment Failure

While preparing the Crude Unit for Turnaround activities a fire occurred. It is assumed that the bolts on the flange of a crude pump opened and products was released. The exact cause is still under investigation, but it is assumed that one of the isolation valves were leaking.
