While performing maintenance activities on the dock, hydraulic hose ruptured on a barge installing pilings.
Machinery is routinely checked to ensure hoses are in acceptable condition prior to each use. Inspection of this machinery before maintenance activities began gave no indications that the hydraulic hose would fail.
River carried away spill before a cleanup crew could be assembled. Maintenance activities were discontinued and the failed hydraulic hose was isolated. This prevented additional hydraulic oil from being released to the river. The failed hose was replaced. Maintenance crews at dock inspected machinery to make sure all hoses were in acceptable condition.
