Cause of Problem: Instrument Failure

loss of accurate level and flow info to the HGU's carbon recovery section resulted in a unit upset by the carrying of hydrocarbon material to the flare system for pressure relief

The level and pressure instrumentation malfunctions had not previously been experienced. Immediate and appropriate were taken by affected process units to address the situation and reduce environmental and operational impacts.
