Written report from Exxon Mobile surrounding the Ammonia Acid Gas flaring incident stating the release of Hydrogen Sulfide due to a leak in the pressure control valve as well as a fault in the wiring system. Report states that wiring pairs have been replaced. An alarm will be placed to alert Console Operator when valve is in the open position to allow Acid gas to flow to flare. Long term improvements to upgrade the control of devices to a more robust control connection. Finally evaluate controles to be managed on a regular interval.
